[pvrusb2] New driver snapshot: pvrusb2-mci-20060102

This snapshot fixes the same initialization problem, this time involving 
saa711x.  Most people probably aren't affected by this, since saa711x only 
appears with 2.6.15 and even then probably everyone is still using 
saa7115.  (If you were trying to use saa711x, then sorry about this...)

Also, last night I finally updated all the web pages to include various 
bits of information learned over the past month.  This snapshot includes 
those new pages in the doc/ subdirectory.
