[pvrusb2] New driver snapshot: pvrusb2-mci-20061003

This snapshot includes a new fwextract.pl to handle the latest driver 
package from Hauppauge (now supporting both 24xxx and 29xxx devices).

There is also technically one driver change, but it's just bug fixes for 
VIDIOC_INT_[S|G]_REGISTER ioctl commands which aren't normally used (a 
debugging aid for me, mainly).

The ala carte downloadable fwextract.pl is also updated (i.e. you don't 
need to grab the entire driver package if all you want is fwextract.pl).  
Note that this is a _further_ update since last week.
